Form anatomy

Each field has a clear purpose.

The form captures enough information to publish an understandable story while leaving non-essential professional details optional.

Required

Name, rating and testimonial

These fields identify the customer, express the overall experience and capture the written story that may be displayed.

Optional

Email, company and job title

Email helps the business identify the submission but is never rendered on public Walls or widgets. Company and title can add context.

Permission

Explicit publication consent

The customer must confirm that the named project may display the testimonial publicly before the submission is accepted.

Designed for the shared link

Branded enough to feel relevant, simple enough to finish.

The page can show the project name and configured logo. Its headline makes clear which business is requesting the testimonial, while the privacy note explains where submitted information goes.

The responsive layout keeps fields readable on a phone, tablet or desktop. When the project is inactive, the collection route no longer accepts public submissions.

  • Project-specific public URL.
  • Responsive one-column form flow.
  • Project logo when configured.
  • Confirmation page after submission.

Comfortable tap targets

Rating, fields and consent remain usable on smaller screens.

Private by default

Submission enters review

The public form does not bypass moderation.

What the form deliberately avoids

No customer account

A customer can respond from the public URL without registering.

No open-ended form builder claims

The fields are purpose-built for testimonials rather than arbitrary surveys.

No automatic publication

Consent enables review; owner approval controls public display.

Photo rules

Text on Free. An optional customer photo on Pro.

Free collection pages do not offer photo upload. Pro pages can accept a validated image stored through TestiFollow’s managed upload flow. The original uploaded filename is not trusted.

If a former Pro account later returns to Free, existing content is not deleted, but new photo submissions are no longer available.

Is the customer email published?
